Job Description

About the role

The Aged Care Worker role is central to our person-centred approach promise which ensures we aim to enhance the psychological, spiritual, social, emotional, and physical well-being of our residents.

Working collaboratively with our nursing team, the role is responsible for providing provide high-quality personal care to our clients based on their individual needs. This includes assisting them with daily life tasks, personal care and mobility. The needs and wishes of individual residents are met as a priority, and we deliver services and supports that ensure the health and wellbeing of our residents.    

Key Responsibilities

  • Provide assistance to clients in accordance with the care plan
  • Carry out personal care needs which shall include daily hygiene, clothing, meals and simple wound dressing.
  • Implement continence programs as identified in the Care Plan.
  • Attend routine urinalysis, blood pressure, temperature and pulse checks, conduct and record blood glucose level checks.
  • Recognise the signs of both Hyper and Hypo-Glycaemia and respond appropriately.
  • Recognise, report and respond appropriately to changes in the condition of clients, within the skills and competence of the employee and the policies and procedures of the organisation.
  • Schedule work programs on a routine and regular basis.
  • Assist in the development and implementation of client care plans.
  • Write required reports and records in the required format in a timely manner and promptly raise any concerns/issues with the supervisor/manager.

Key Knowledge and Experience

  • Minimum Certificate III in Individual Support (Ageing) or in Aged Care.
  • Experience working in residential aged care (preferred).
  • Clear verbal and written communication skills.
  • Ability to provide quality care and exceptional service.
  • Passion for enhancing the lives of older people and for working in aged care.
  • An understanding of the needs of people living with dementia.
  • Respectful approach that supports people based on their choices.
  • Ability to work collaboratively within a team, as well as the confidence to work independently.
